By joining Perfect Health Sweden, a private health club, you will have access to information on proper diet, botanical regimens, daily routines, and information on common diseases. Recommendations for botanical treatments are based on your health history, blood work, and iridology readings. Paid memberships to the club are non-refundable but can be downgraded, paused, or cancelled at any time. By entering this agreement, you acknowledge and accept the following terms of membership:

• You understand that you have the right to pursue various health practices and consult with whomever you choose to achieve your goals for better health.
• You understand that the representatives of Perfect Health Sweden are not medical doctors, but rather herbalists, crystal and reiki healers. Only a medical doctor can diagnose and prescribe treatment.
• You give the representatives of Perfect Health Sweden permission to openly discuss your personal health concerns and potential solutions to those issues. They may provide educational information, including but not limited to fasting, herbal botanicals, diet, and nutrition, but this is not a substitute for medically necessary diagnostics and does not include diagnosing or prescribing treatment.
• You agree to not file any malpractice lawsuits against any member of Perfect Health Sweden.
• You agree not to share any information about your actions with Perfect Health Sweden with any state or federal regulatory boards, the FDA, FTC, Medicaid, or any other insurance company without permission. You also waive your HIPAA privacy rights and complaint processes.
• You understand that no doctor-patient relationship exists between you and Perfect Health Sweden. It is your responsibility to educate yourself on any advice or recommendations given to you by a member of Perfect Health Sweden.
• Perfect Health Sweden declares that all product recommendations for reversing illness and mineral deficiency in the body are only recommendations. These products are considered supplements and are not cures for conditions such as herpes, cancer, diabetes, autoimmune diseases, and others. However, with proper diet, these supplements may have the ability to improve the immune system, nervous system, and lymphatic system. The Food and Drug Administration has not evaluated statements made about specific supplements and does not recognize that any nutritional supplement can be used as a cure for any condition.
